Make sure you saved each of the forms-- bonus points* if you rename the form with your name and the form title (ex Garcia, Mabel - W4).
Open your email account and click "Reply" to the "Welcome to MSCR" email from your Program Assistant.
In Gmail and Yahoo mail: click/tap the paperclip icon to attach the document.
Using Apple's mail app: click/tap the paper with a folded corner icon to attach the document.
All other email accounts: hover over the icons until you see "Attach."
Find your saved files, select the forms you have completed.
Click send!
Please make sure to use blue/black ink on the forms and make sure it's legible. Any forms we cannot read clearly will need to be redone.
You have 3 options to submit the forms:
Scan your completed forms and follow the "completed forms digitally" instructions above.
Take a good, high quality photo of your completed forms (one form in each picture) and follow the "completed forms digitally" instructions above.
Bring the completed forms to your Paperwork Appointment.
