Let me know if you are taking the course for Fitchburg State credit (and submit registration & payment to FSU by course due date)
Make certain you have and can access your @lunenburgschools.net Google account BEFORE the course starts!!
Submit to your administrator the required Employee Notification and Request Form (parts to fill out are outlined below) and keep a copy for your records
Professional development courses offered through Lunenburg Public Schools may be taken for Lunenburg credit at no cost. These in-house course credits may be applied to degree status advancement solely in Lunenburg. See Article XXI Course and Conference Reimbursement of the teacher contract for more information.
Professional development courses offered through the district also offer an option to obtain Fitchburg State University credit for an additional fee. The cost for each course will vary depending on number of credits and year offered. Each course will have a unique schedule number and course number that is needed to register. Information for approved courses will be sent district-wide when received from Fitchburg State University. A history of recent courses can also be found on the PD Course Offerings page.
If you are taking a course for in-house credit only:
fill out the Employee Notification and Request Form and submit it to your administrator
If you are taking a course for Fitchburg State University credit:
fill out the Employee Notification and Request Form and submit it to your administrator
register for the course and submit payment to FSU (information below)
If taking a course for FSU credit, fill out sections A, C & E
If taking for in-house credit or PDPs, fill out A, D & E. Please note in D where it says course cost instead of 0, you should note taking for PDPs.

In addition to filling out the Employee Notification and Request form (see above), you also need to submit a course registration form to Fitchburg State University. A blank Fitchburg State Course registration form is attached below.
You must register and submit payment for each course to FSU by the date outlined in the course approval form.
Teachers seeking course reimbursement should see Article XXI Course and Conference Reimbursement of the teacher contract for more information.
Students will need the schedule number and course number to register. Students should print the registration form to fax or mail it in. More course information may be found at www.fitchburgstate.edu/cps. Forms with missing or incorrect information may be returned and delay processing time.
Full payment is required when the student registers for the course. Students may submit personal checks (made payable to Fitchburg State University), VISA, MasterCard, Discover or American Express as forms of payment. Cash and purchase orders will not be accepted.
