Paperwork

Please fill out required paperwork, follow steps 1-4 below by clicking on each of the underlined options. All paperwork must be completed prior to practicing.

All paperwork should be turned in no later than AUGUST 8th!!

Step 1: Athletes, sign up for google classroom and complete the Player Contact Information.

Step 2: Complete your Physical Paperwork AND Health History Paperwork. This is to be turn into the main office.

Step 3: Complete the Genesis registration

Step 4: Pay your Participation Fee after the tryout period.
