Mission Statement

The Northside Health Center is committed to providing exemplary comprehensive health care to the students at Northside High School. Physicals and psycho social services will be provided by a dedicated staff member in an informal and confidential atmosphere that is easily accessible to all students. Emphasis is placed on wellness, avoiding high risk behaviors, and meeting the individual medical, emotional, and social needs of adolescents, at no cost to the student. The Health Center will not distribute, prescribe, nor refer for prescription of contraceptives. The health center will not counsel for abortion nor refer for abortions.

We believe in the absolute promotion of a healthy life that includes:

  • Communicating in a positive manner with those important to your life.
  • Routine wellness physicals and sports physicals.
  • Eating a healthy, balance diet.
  • Learning to make responsible decisions about drugs, alcohol, and abstinence.

The Northside Health Center provides quality healthcare and wellness services for our students, with an aim to enhance their experience on campus. Our staff provides non-judgmental care and mutual respect to all clients. We protect right to privacy in delivering health services to all students and faculty seen at the clinic.

The health clinic is staffed by well-trained and caring individuals. The staff consists of a Family Nurse Practitioner, Registered Nurse, Medical Support Assistant, Social Worker, and a Clinical Dietitian.

The link above provides access to forms needed for a student to successfully enroll into the Northside High School Health Center. All students enrolled into the health center are permitted to utilize it and its services during their matriculation here at Northside. All forms should be read and fully completed before returning to the health center including signatures from guardians as needed. The Minor Consent Law document is attached and should not be returned as this form is used to provide information about the rights of children receiving the health center’s services. Also, the Notice of Privacy Practice is a document that should be read, signed, and the final page returned along with the consent form. There are a wide range of students who are already enrolled in the health center and may have to update some of their demographic information. If so, the Annual Update Form is available for them to complete and return.
