Placement review forms
Please be careful to select the appropriate form based on your current school and the type of request.
Forms for students currently in grade 8 at Clarke or Diamond
DEADLINE for all of the following forms: March 15, 2024.
Math Placement Review: Appeal form for students who want to take a 9th grade math course that they have not been placed into by their teacher.
Math Placement Review: Extra Acceleration form for students who want to take a placement test to go into Honors Math 3 or higher.
Forms for students currently at LHS
DEADLINE for all of the following forms: March 15, 2024.
Math Placement Review: Appeal form is for all placement reviews except for when a student is proposing to skip over an entire course.
Math Placement Review: Acceleration form is for these kinds of requests to skip a course:
grade 9 currently in Math 1, asking to take Math 3 next
grade 10 currently in Math 2, asking to take Precalculus next
grade 11 currently in Math 3, asking to take Calculus next
Math Placement Review: Extra Acceleration form is for these kinds of requests to skip a course:
grade 9 currently in Honors Math 2, asking to take Honors Precalculus next
grade 10 currently in Honors Math 3, asking to take AP Calculus BC next
