  • Personal Absence Request - Need a little R&R? You'll need to fill this out whenever you need to take a personal day.

  • Substitute Evaluation Report - If you have had an incident with a sub and you would like to have Ms. Gifford address it, fill out this form and bring it to the AP Office. Call Anne-Marie Hector at x11816 if you have any questions regarding the form or process.

  • Work-Related Absence Request - This must be completed whenever you are out for staff development. Please do this several days before your absence so it has time to get approved.
