  • Personal Absence Request - Need a little R&R? You'll need to fill this out whenever you need to take a personal day.

  • Substitute Evaluation Report - If you have had an incident with a sub and you would like to have Mr. Boehringer address it, fill out this form and bring it to the AP Office. Call Micah Gillum at x11816 if you have any questions regarding the form or process.

  • Work-Related Absence Request - This must be completed whenever you are out for staff development. Please do this several days before your absence so it has time to get approved.

Need a Staff Parking Permit? - Submit this form and one will be delivered to your teacher's box.

Ranger Tech Repair Request - Do you need Melokka's Ranger Tech kids to come and trouble shoot a computer issue? 

Work Orders - 

If it's a ROUTINE matter (ex: lightbulb out):

• submit the Maintenance Request form 

If it's an URGENT matter (for ex: toilet overflowing, no electricity):

• contact Receptionist at 61800 or the AP Administrative Office at 11820/11816

• submit the Maintenance Request form 

If it's an HVAC issue:

• contact Courtney Tully at 11811 or via email

• submit the Maintenance Request form
