Dear Wiley Parents/Guardians,
Welcome - My name is Alicia Esposito, BSN, RN and I am so excited to be your campus nurse! I would like to take this opportunity to share some information with you on ways we can partner to make the school year successful.
Medications: For the safety of our students, all medications are kept in the clinic. All clinic forms can be found through the links below OR on the Leander ISD Health Services webpage.
Illness/Hospitalization: If your child is sent home with fever, vomiting, diarrhea or other contagious illness, they must be symptom free for 24 hours without the use of medication before returning to school. If your child has been prescribed antibiotics, they must receive the medication for 24 hours prior to returning to school.
If your child has been hospitalized, a note from the student’s physician stating the date the student is allowed to return to school is required. Students who come to school with an injury requiring interventions such as surgery, casts, crutches, wheel chairs, etc., must have a physician’s note prescribing the medical device (crutches, wheelchair, cane, walker, etc.) and what activities the student is/is not allowed to do while under the physician’s care for the injury.
Immunizations: Please send your child’s updated immunization record(s) to the clinic as soon as possible. If the updated record is not received on or before the exclusion date determined by Leander ISD and as required by State Law, your child will be excluded from school.
State Mandated Screenings: The following state mandated screenings will be performed according to the Texas DSHS guidelines:
Hearing and Vision
7th Grade
All students new to Texas schools
Acanthosis Nigricans (physical skin finding that may be a marker for high blood insulin levels)
7th Grade
Spinal
Females in 7th Grade
Males in 8th Grade.
*If your student has been screened by a physician please provide documentation from the physician to the Nurse*
Student Health Information Changes: If your child’s medical condition changes at any time, please submit a Student Health Information Update.

Administration of OVER-THE-COUNTER medications during the school day REQUIRE a medical provider AND parent/guardian signature.
This includes but is not limited to: Ibuprofen or acetaminophen, oral or topical allergy medication, insect bite or burn-gel.
Does your child need to take a medication daily during school hours?
Does your student have an allergy that may cause anaphylaxis?
Does your student need an inhaler or Emergency medication for seizures?

Please download and print the appropriate forms through the links above. Bring the completed forms signed by your child's Primary Care Provider with you when you bring medication to the Wiley Middle School Clinic.
The best time for medication check in is in the morning OR scheduled with the Nurse in advance. At all other times there may be a significant wait time to check in medications.
**Per Leander ISD policy you may not send any medication with your child to school**

The clinic is located in the main hallway directly behind the front office. For safety and privacy the door will be shut and the doorbell will need to be used to gain access.
