Complete the Field Trip Request form (Link on right). Ensure that all students have filled out a Regular/Extended Day Permission Form or Overnight Permission Form. Please ensure all information is complete and accurate and remember to have all forms with you the day of the trip.
After completing the Field Trip Request, submit class rosters (either electronically or paper copy) to Sarah Larson, school nurse. She will identify students with medical needs
It is your responsibility to obtain medical training (allergies, diabetes...etc.). If this is an overnight field trip, additional forms will be provided by the nurse for medications. You may be responsible for dispensing medications.
Before the bookkeeper can take any money or make reservations, all field trips must be APPROVED by Transportation. To track the status of your trip access the Field Trip Calendar 2020-2021.
Your club/activity is responsible for the total cost of the field trip. This includes any associated fees such as tickets, meals etc. and the cost of the bus. Once your field trip is approved, Transportation will send a confirmation and cost estimate to Mr. Tim Yorke. He will notify you of the approval and the estimated transportation cost.
At least one week before the trip, send an 'ALL-STAFF' e-mail notifying teachers and staff of your field trip. Please include an alphabetized (by last name) list of students along with the date(s) and time(s) of the trip. Teachers may request that a student in poor academic standing remain in school.
1. Field trips must be turned in 5 weeks before the scheduled trip. The earlier you schedule a trip, the greater likelihood you'll have obtaining a bus. Buses are limited; plan ahead!
2. Requests for trips which cause students to be absent from school more than five days or involve travel outside of the Continental United States must be approved by the Assistant Superintendent for Instruction. Copies of such requests approved by the principal shall be forwarded at least eight weeks in advance for approval. See the Foreign Field Trip Organizer's Guide (above).
3. If you're sharing a bus with another school, each school requiring transportation is required to submit a Field Trip Request; the transportation department will consolidate shared trips as applicable.
4. County policy stipulates one chaperone per 10 students. Chaperones are limited to LCPS staff and parents of students attending the field trip. The pulling of instructional staff should and will be limited. Remind staff members attending the trip to request a substitute if necessary.
5. Overnight trips require both a male and female chaperone if both male and female students are attending.
6. Canceling a trip or altering the number of buses MUST be done at least two weeks in advance of the trip. Failure to do so may result in assuming full financial responsibility for the cost of the canceled buses (per LCPS Transportation Director.
7. Field trips cannot be scheduled after March 31 because of SOL testing. Exceptions can only be made with Mr. Adam's approval.

9. Bus drivers on field trips are instructed to travel together and drivers have the final say as to the route of travel.
10. Teachers and chaperones traveling on field trips are required to maintain discipline on buses. All passengers must remain seated whenever the bus is in motion and noise should be kept within reasonable limits. Drivers have the authority to maintain discipline and/or assign seats to students exhibiting inappropriate behaviors that create a distraction to the driver or a hazard to the safety of the passengers.
11. The sponsoring school is responsible for providing parking fees, tolls (outside of Northern Virginia) and on overnight trips, lodging for the bus driver.
Travel outside the contiguous states in conjunction with instruction on the high school level can provide excellent educational experiences for high school students since it enriches and expands the students’ classroom experiences and provides opportunities which enhance studies in any discipline. Foreign trips shall be considered a school-sponsored activity with all rules and standards for student conduct applicable.
No School Board or school funds will be committed to underwrite a trip or to reimburse individuals for financial loss due to trip cancellation or modification for loss of money or possessions during the trip. The principal of the teacher/coordinator’s school shall, as he/she deems necessary or unavoidable, have the authority to effect changes in trip itinerary, cancel the trip, and institute procedures not otherwise covered in the Administrative Guidelines.
Students who have graduated from high school are not eligible to participate in foreign trips. Each student’s principal has the authority to deny eligibility for a foreign field trip.
Any prescription medication which needs to be administered on an extended day or overnight field trip must have the “Authorization for Medication Administration” form completed and signed by the parent and physician on file in the health office. Medication for these field trips must be supplied in an original pharmacy-labeled container with the amount of medication required for the field trip.
Extended Day Field Trip Medications Medication The parent/guardian of a high school student who is participating in an overnight or foreign field trip has the option of signing permission to authorize the child to carry and self-administer either over-the-counter or prescription medication when the parent/guardian accepts complete responsibility for both the child’s decision and actions while on the overnight or foreign trip. The “Universal Permission Form” for Overnight and Foreign Field Trips must be completed and submitted to the school nurse with completed “Authorization for Medication Administration” paperwork for each medication no later than two weeks prior to the departure date of the field trip. Parents must supply both the over-the-counter and the prescription medication for all overnight or foreign field trips, even if the parent does not choose to have the child carry and self-administer the medication. Medication will not be provided from the health office for overnight and foreign field trips. Overthe-counter medication must be stored in the original manufacturer’s container with no more medication than is required for the duration of the field trip. Prescription medication must be stored in the pharmacy-dispensed and labeled prescription container with no more than what is required for the duration of the field trips.
