Rank One (7 forms)
Online forms must be completed using the Rank One program for all students. Make sure when you are filling out the online forms that you have selected the 2026-2027 school year.
Students CANNOT participate in band camp without a completed physical. The physical must be dated after April 1st, 2026 and be signed and dated by the student, parent, and healthcare provider. It is recommended to keep your paper copy of your physical in case there are any questions.** Click here for a blank physical form.
COLOR GUARD MEMBERS: Please choose both Color Guard AND Band when filling out Rank One Forms.
The Drug Testing Consent form is NOT completed in Rank One. That is the separate form listed below. We will update Rank One when the Google form is submitted so that the drug testing form shows as complete.
Medication Administration This form allows band staff to dispense over-the-counter medication to your student should your student request it. Please complete this form even if you do not want any medication dispensed to your student.
Band Handbook Acknowledgement Form This form verifies that you and your band student(s) have read and understand the policies and expectations for the 2026-2027 band program.
Drug Testing Consent All LTISD students who participate in extracurricular activities are required to participate in the Student Drug Testing program.

Custom Meals Request (optional) This form lets us know if a student has special dietary needs. If they do not have any of the listed special dietary needs, you do not need to complete this form.
