Bereavement Leave
An employee will be allowed leave with pay for a maximum of three (3) working days for each occurrence of a death in the employee’s immediate family. The definition of an immediate family member shall include the father, father-in-law, mother, mother-in-law, brother, sister, spouse, child, grandparents, grandchildren, step-parents, step-children, great-grandparents, foster parents, foster children, a domestic partner, any relative who resided in the employee's household, a household member (any person residing in the immediate household of the employee at the time of death) and the following relatives of an employee’s domestic partner: child, grandchild, mother, father.
The employee shall be entitled to use this leave up to 370 days from the date of the death of the qualifying family member. Bereavement Leave not used before 370 calendar days from the date of said death shall be deemed waived and lost.
Please use the code BL when submitting your Time Entry. You will also need to provide a published obituary, funeral program, prayer card, etc., once you are back from bereavement leave to Nancy Chean or Matthew Chua.

Disposition of your last paycheck upon resignation and/or retirement
Based on the City's practice, your final paycheck for any remaining unused time on the book, upon applicable, will be issued in the following pay period, following the last pay period you were active on the payroll.
[The final paycheck will be a "hardcopy" check, not direct deposit.]
Example: If termination is effective for 12/3/22, your final paycheck for any remaining unused time on the book will be issued for the 12/28/22 payday.
Important Notes: Please keep in mind that we are two weeks behind in our pay. The 12/14/22 payday is for the pay period ending 12/3/22 time.
FICA Medicare Excess (Code 07E)
The FICA medicare excess deduction (0.9%) will occur once you exceed the $200, 000 YTD salary cap. Please keep in mind this deduction will continue to increase as your salary keeps going up.
Jury Duty
If you are summoned for Jury Service, please notify your Supervisor which day(s) you will be off immediately.
Please use the code JD when submitting your timesheet. You will also need to provide proof of your service (the notice of jury duty service or summons, or "Certification of Jury Service" notice provided by the court) as soon as it has been completed to Nancy Chean or Matthew Chua.
Important notes:
Full-time City employees will be compensated for time spent on jury service during regularly scheduled workdays, up to the number of scheduled work hours, but no more than eight hours a day.
City employees are not authorized to waive jury fees and will be required to remit the appropriate fees to the City upon completion of service.
Mileage allowances may be waived at the option of the employee. Any mileage allowance paid by the Court should be retained by the employee.
Employees are not required to pay the City those attendance fees received for jury service on their regular day off or on a holiday.
Lost your "hardcopy" paycheck
Please notify Payroll ASAP, so we can submit a Stop Payment request to the Controller's office. We will then forward you the Affidavit to fill out, and send it back to us for processing.
Name Change
In the event that you need to change your name, please submit the required documents onto Workday. You will also need to notify Human Resources so that your work email can be updated. [Required documents: A copy of your Social Security card with the new name or a copy of your receipt from Social Security.]
