Use the form to submit required audit reports to LDOE.

The annual audit report submitted must contain the same documents required as part of the ‘audit reporting package’ as defined by the ‘State Audit Law’ and Louisiana Legislative Auditor guidance. This should include, but not be limited to, the Comprehensive Annual Financial Report (CAFR); Single Audit Reporting Package; management letter(s), if any; and, for school districts and charter schools, assurances required for performance and statistical data provided as part of the financial statement audit.

You will need a google sign-on to perform this task.

Use the form to transmit reports (in .pdf format) as required. Fill out a form for each report.
