Our new Population Health Platform (Persivia) optimize processes related to population health management and value-based care, facilitating the identification of key populations and patients for management, streamlining care management activities, presentation of care gap opportunities within eCW and tracking/reporting on key metrics.

Implementation of the Population Health Platform (Persivia) will be by phases integrating medical and financial data related to inform care management and other modules. Florida will be the first market to go live starting on January 16th and then it will move to other states.
