Schedule:

・First period: March 22 (Tues.) to March 31 (Thurs.) *10-days straight including weekend

・Second period: May 21 (Sat.), May 28 (Sat.)

 Based on the results of the first period and the surrounding circumstances, we have cancelled the scheduled vaccinations on May 22 (Sun.) and 29 (Sun.).   

 We apologize to those who had hoped to receive vaccinations on these Sundays.

Below is an overview of the second booster vaccination period.

5F South School Building, Mita Campus, Keio University

Eligibility:

Persons aged 18-65 years of age or older for whom at least six months have elapsed since they received their second dose of the vaccine and who are in possession of a 3rd vaccination voucher sent by their local authorities (vaccination is non-mandatory.)

 ・Undergraduate students, graduate school students, and Correspondence Course students

 ・Faculty and staff members and their cohabiting family members

 ・Persons who work on a Keio University campus, including some contractors

Only those who have previously received two doses of the COVID-19 vaccine are eligible for booster vaccinations.

It is your responsibility to confirm how many doses you have already received. Please only make a reservation if this will be your third inoculation.


If you received your first (1st or/and 2nd) vaccination overseas, please be sure to check the Ministry of Health, Labor and Welfare website, and if you have any questions, please contact the "Call Center" listed at the bottom of the page.

Vaccination Appointments

Second period: Online reservations will open Monday, April 18, at noon.

*It will be possible to log into the system early beginning at 11:30AM that same day.

*Please have your vaccination voucher number ready on hand if you did not receive both vaccinations at Keio University in 2021 (i.e. those who received one or more vaccinations at venues other than Keio University) as it will be necessary to enter this number when making a reservation.

Appointments will be scheduled between 12:45-16:30 (15-minute time slots) each day of the vaccination periods.
The estimated time required from checking in until receiving the vaccination, including waiting times, will be approximately 30 to 60 minutes.
It may take some time from checking in until the vaccination is finished. Please be sure that you have plenty of time to spare before making an appointment.
Please note that Keio’s system is not linked with the reservation system for vaccinations administered by local authorities or the Japanese government.
If you have made an appointment for vaccination at both another venue and Keio University, be sure to cancel one of the appointments.
You can cancel your Keio vaccination through the system listed above.

What to bring on the day of your vaccination

Everyone

・The registration confirmation slip for the day of your vaccination

You will need the bar code for your appointment as generated by the Keio Vaccination Reservation Management System. You can either print it out on A4 size paper and bring it with you or present it to staff at the venue via the screen of a smart phone or other electronic device.


・A form of identificationi.e. student ID card (students who will graduate or complete programs in March 2022 can use their student ID card with a hole punched in it), faculty/staff ID, driver's license, “My Number Card”, insurance card, passport, residence card, or other form of personal identification.


Those who have received a vaccination voucher from their local municipality

・Prevaccination Screening Questionnaire and COVID-19 vaccination certificate received from one’s local municipality


There are two types (differing by municipality) of Prevaccination Screening Questionnaires for booster vaccinations:

(1) Prevaccination Screening Questionnaires that are combined with a vaccination voucher (voucher is printed on the questionnaire)

(2) Prevaccination Screening Questionnaires that are separate from a vaccination voucher (voucher is not printed on the questionnaire)


For those with Prevaccination Screening Questionnaires that are separate from a vaccination voucher (2), please also bring your vaccination voucher


Vaccination certificates also come in two varieties, either combined with or separated from the vaccination voucher, depending on the municipality.

Be sure to print each single sided on A4 paper. Documents printed on other sizes of paper or printed double sided will be deemed invalid.

Instructions on how to fill out the Prevaccination Screening Questionnaire:

  1. Do not use erasable ballpoint pens.

  2. Fill out all the information in the bolded boxes before coming to the venue.

Checklist:

  • Have you written your name and address as it appears on your Certificate of Residence?

  • Have you filled out your phone number, date of birth, age, and gender?

  • Have you filled out your temperature as taken at home on the day of your reservation?

  • Have you left any of the questions section or response boxes blank?

  • Have you checked the "I wish to be vaccinated" box?

  • Have you filled in the date of inoculation?

  • Have you signed your name in the signature-box for the person to be vaccinated?

Frequently Asked Questions (updated April 15)


Q: Will I have to pay to get vaccinated?

A: No. There is no fee for vaccination.

Q. When can I make an appointment for a booster vaccination (3rd inoculation) using the university-based Keio program? (Red portions were amended on April. 4, 2022)

A: Reservations for the first period (March) began at noon on Friday, March 11. Reservations for the second period (May) will begin at noon on Monday, April 18. (Tentative)

Q: If I am a student graduating/completing my program in March of 2022, will I be eligible to get my booster vaccination (3rd inoculation) through Keio University?

A: Yes. If possible, please get vaccinated during the first period (March).

Q: I am a student who got both my first and second vaccinations at Keio University and graduated/completed my program in September 2021. Am I eligible to get my booster vaccination (3rd inoculation) at Keio University in 2022?

A: It will not be possible for you to receive your 3rd vaccination at Keio University.

Q: I am a student currently taking a temporary leave of absence. Am I eligible to get my booster vaccination (3rd inoculation) at Keio University?

A: Yes.

Q: Are faculty, staff, and their cohabiting family members eligible for booster vaccinations (3rd inoculations) at Keio University if they are retired, planning on retiring, or have not yet begun to work? (Red portions were amended on April. 15, 2022)

A. Faculty and staff members on the payroll as of March 1, 2022, and their cohabitating family members, as well as faculty and staff members scheduled to start work between April and May 2022 and their cohabitating family members, are eligible to apply.

 Please note that cohabitating family members of faculty and staff members scheduled to start work between April and May 2022 will, in principle, be eligible for vaccinations beginning in the second period (May).

 Faculty and staff members on the payroll as of March 1, 2022, and their cohabitating family members, as well as faculty and staff members on the payroll as of May 1, 2022 and their cohabitating family members, are eligible to apply.

Faculty members scheduled to start work in May 2022 can make an appointment via Keio Vaccination Reservation Management System from noon on May 1. *Family members living together cannot make reservations this period.

Q: I completed my first and second vaccinations at another location. Would I still be eligible to receive my booster vaccination (3rd inoculation) through Keio University?

A: Undergraduate students, graduate students, correspondence course students, faculty (including part-time lecturers), staff members, a portion of contracted employees, family members who live with a faculty/staff member, etc., who have received their voucher for a booster vaccination (3rd inoculation) from their local government are eligible for the university-based vaccination program when presenting their voucher at Keio University.

Q: I am a faculty/staff member. I have a child who lives with me but is not my dependent. Are they eligible for a booster vaccination (3rd inoculation) at Keio University?

A: For cohabiting family members of faculty/staff, we will not ask whether they are dependents or not. If the family member is 18 to 65 years old, they are eligible.

Q. Do "cohabitating family members of a faculty/staff member" need to have the same address as the faculty/staff member listed on their Certificate of Residence to be eligible?

A. In general, family members who live together on a regular basis are eligible for vaccination.

Q: Are family members of students eligible for booster vaccinations (3rd inoculations) at Keio University?

A. Family members of students are not eligible for vaccinations through Keio University.

Q. I have not yet completed my first two vaccinations. Can I receive them in 2022 through Keio’s university-based vaccination program?

A: Keio’s 2022 university-based vaccination program is only meant for those seeking booster vaccinations (3rd inoculations).

Q. I am unable to bring my booster vaccination voucher. Am I eligible to get my booster vaccination (3rd inoculation) through Keio University? (Amended Mar. 1, 2022)

A. As a general rule, only those who bring their voucher are eligible for a booster vaccination. If you have not received a booster vaccination voucher from your local government office, please first request the local government to issue a booster vaccination voucher so that you can bring it with you on the day of your inoculation.

(If you have received your 1st and 2nd vaccinations at Keio University but have not submitted your 1st and 2nd vaccination vouchers to Keio, your local government office will not issue a booster vaccination voucher. Please submit your 1st and 2nd vaccination vouchers to Keio University as soon as possible. Please follow the guidelines listed under the section “for those who have not submitted vaccination coupons” on the “After COVID-19 Vaccination” website for instructions on how to submit vouchers.)

If you request a 3rd vaccination voucher from your local government and it does not arrive by the day of your booster appointment, you can receive your booster vaccination at Keio University without a voucher only if you received your 1st and 2nd vaccinations at Keio University in 2021 and have already submitted your 1st and 2nd vaccination tickets to Keio University.

In such a case, please be sure to submit the third vaccination voucher (or the prevaccination screening questionnaire if the vaccination voucher is attached to it) received from your local government office to Keio University at a later date.

Q. In 2021, I received my first and second vaccinations at Keio University without bringing my vaccination vouchers. Afterwards I forgot to submit them to Keio University. What should I do now?

A. Please follow the guidelines listed under the section for those who have not submitted vaccination coupons on the “After COVID-19 Vaccination” website and complete the necessary procedures as soon as possible.


Q: I never responded to the advance survey sent out in January 2022, but am I still eligible to get my booster vaccination (3rd inoculation) at Keio University? (Added 2/21/2022)

A: Even if you did not fill out the survey, if you meet all the other requirements, you are eligible for vaccination.


Q: I answered "I would like to be vaccinated at Keio University" in the advance questionnaire that was conducted in January 2022, but can I receive the vaccine at another venue? If so, do I need to inform Keio that I have changed my mind? (Added 2/21/2022)

A: You are welcome to get your vaccination through another venue and you do not need to contact us if there are changes to your survey response.

Q: I would like to get the vaccine produced by Pfizer, Inc. Can I choose this vaccine instead?

A: No, you are not able to choose which vaccine you will receive. All vaccines administered at Keio University will be the Moderna vaccine.

Q: I am a student. If I suffer any side effects, will this be taken into consideration with regard to my class attendance?

A: There are no measures in place to exempt students from attending classes due to the side effects of the COVID-19 vaccine. When making a reservation to be vaccinated, please do so with your class schedule and the possibility of side effects in mind. If you are forced to miss any classes due to side effects of the COVID-19 vaccine, please individually consult the faculty members in charge.

Q: I made an appointment but feel unwell on the morning of the vaccination. What should I do now?

Please cancel your appointment and monitor your ongoing condition. Then complete another reservation for a different date and time to be vaccinated.

Q: If a friend/coworker has an appointment but is unable to attend can I get vaccinated in their place?

A: Only the person who completed the reservation may be vaccinated. The person who made the reservation should ensure to come to the vaccination venue. In addition, please ensure to cancel your appointment if there is a scheduling conflict or if you feel unwell.

Q: Right before I get vaccinated I am planning on moving/changing the address on my Certificate of Residence. How should I enter my address on the prevaccination screening questionnaire?

A. Please contact the local government where you currently reside to find out which municipality to write down and information pertaining to vaccine vouchers.

Q: Are vaccinations compulsory at Keio University? Will I be prohibited from entering the campus or be subject to other restrictions related to classes, extracurricular activities, or work if I do not get vaccinated?

A: Vaccinations are administered at the discretion of individuals. As such, getting a vaccination is not compulsory. In addition, vaccination will in no circumstances be made a prerequisite for activities or taking courses on campus, or for performance of work duties.

In Preparation for University-based Vaccinations for COVID-19 (3rd Dose)

Keio University Infection Response Center for COVID-19

In advance of the beginning of the new semester on April 1, 2022, Keio University is providing the opportunity for university-based vaccinations (3rd dose) for COVID-19 at Mita Campus from March 22, in the same manner as last summer. On this occasion we will once again, in cooperation with Keio University Hospital, Saiseikai Central Hospital, and other medical institutions, put in place a steadfast system to ensure that anyone who wishes to be vaccinated can do so safely.

Cumulative data on the 6th wave of infections in Japan has shown the rate of severe illness and fatalities attributable to COVID-19 infections in children under the age of 10, teens, and younger people in their 20s to be extremely low. In addition, it has been reported that, in general, a third dose of the vaccination boosts previous protections against the development symptomatic infection and severe illness.

While vaccines offer benefits, all come with side effects, and there remain areas requiring ongoing investigation. These include the long-term effects of COVID-19 infection as well as the mRNA vaccines themselves. It can also be assumed that the extent to which vaccination is needed will vary depending on an individual’s age, the presence or absence of underlying conditions, their physical constitution, living and learning environments, and their participation in extracurricular activities. Further, it remains of utmost importance to comply with the required codes of conduct and to institute preventative measures to mitigate the spread of infection to others in your environment, regardless of whether or not you have been vaccinated.

 As was the case during the 1st and 2nd dose vaccinations last summer, the 3rd vaccination is optional and Keio University will not impose any restrictions on the activities or movements of faculty, staff members, and students based on whether or not they have been vaccinated. We entreat each individual to exercise their well-considered discretion when making a decision on the 3rd vaccination.
