MCHS Physical Night- may 5, 2026
Medical Colleagues of Texas will be performing physical exams at Mayde Creek High School for $30 and we will be providing transportation to and from Mayde Creek High School.
Students that choose to participate will stay after school in the gym until the bus leaves Cardiff around 4:30. Students will be transported to MCHS where the physical will take place. We do not have an exact time as to when we will return, but our best guess is 7:00pm. Please remember, this is not exact! It could be earlier; it could be later.
We have set up a Sports You Group so that we can stay connected with parents throughout the night and we will also allow students to have their phones. Please be ready to pick students up 10-15 minutes after getting the phone call or text that we are headed back to Cardiff.
IMPORTANT- All parents that choose for their students to ride the bus to and from Cardiff to Mayde Creek High school for physical night must complete the following items by Monday, May 4:
Permission slip signed by parent/guardian
Confirmation of Understanding of Scope and Purpose of Pre-Participation Physical Exam
Medical History form completed and signed by parent/guardian (please make sure to sign the bottom of the medical history form)
Top of the Physical Examination form completed (student’s name, sex, age, date of birth)
$30 payment- cash, check, or online at https://katyisd.revtrak.net/athletics-physicals (Payment link opening soon)

***If your student cannot participate in this physical night, there are several others the district is offering. The dates and times are attached to the medical history/physical form which can be found below. If you prefer to use your own doctor, please remember that all physicals must be dated on or after May 1, 2026.
