Preschool Registration Requirements

First time in a Hawaii Public School:

• Requirement by First Day of School  (click to see requirements)

  • Tuberculosis (TB) skin test within one year of  entry into Hawaii schools.

    • Available for free at the Department of Health, 3040 Umi Street, Lihue HI 96766- call 241-3387 for an appointment 

  • Original Birth Certificate (Copy will be made by school staff member)

  • Proof of Residency in our district: Utility bill with residential service address or Rental contract. (If living with a family, a copy of family member’s utility bill showing address and their signed statement that you are living with them.)

  • Physical within one (1) year  or  appointment card from a physician’s office showing date and time of physical appointment. 

  • Immunization record
