Parent/Guardian must complete BOTH steps:
Step 1: Submit the Summer School APPLICATION
Step 2: Download, sign, and return the SIS-10W to the school's Registrar's Office (F106). If you cannot print, you may come to F106 to complete the SIS-10W form.
Please bring a valid photo ID.
If you have a new physical address, proof of current residency is required.
IF YOU DON'T COMPLETE THE SIS-10W, YOU ARE NOT FULLY ENROLLED!
You will receive an email confirming your enrollment in the Summer Program.
