Form Builder understands and converts your Google Doc and Google Slides into a form based on specific formatting rules. Follow these carefully to get the best results:
Title Style:
If text is formatted as Title, it will be added as a Form Title.
Heading Styles:
Heading 1 / Heading 2 → Converted into Sections
Heading 3 → Converted into Title + Description
Question + Options (Multiple Choice):
If a paragraph or line of text is followed by bulleted or numbered items, Form Builder treats:
The text → as the question
The bullet/numbered items → as answer options (Multiple Choice type)
Multiple Correct Answers (Checkboxes):
If more than one option is bold, it will:
Treat all bold options as correct answers
Automatically set the question type to Checkboxes
Simple Questions (Short Answer):
If a question does not have any bullet/numbered options, it will be treated as a Short Answer question.
Any images in the Google Doc will be automatically added as images in the Google Form.
Correct Answer (Quiz Mode):
If any option is bold, it is marked as the correct answer, and the form is automatically set as a quiz.
Required Questions:
Make the question text bold → it will be marked as required in the form.
If the last option in Multiple Choice is written as OTHER (in capital letters):
The “Other” option will be automatically enabled in the form.
Once your document is loaded into the sidebar, you can further customize each field.

Section
Click the (Settings/Gear icon) to access advanced customization options for your form.
Read the rules carefully before creating your document.
Do NOT use bullets or numbering for questions.
Use bullets or numbering ONLY for answer options
(Otherwise, options won’t be detected correctly).
To include an “Other” option, always use OTHER in uppercase as the last choice.
To mark a correct answer, make that option bold.
Content inside Google Doc tables will be treated as questions only.
Answer options inside tables will NOT be recognized as options.
