All required participation forms, including the KHSAA MS Sports Physical, must be 

uploaded to FinalForms.  Once you create your account, you will only need to 

upload a new physical each year and make sure all of the basic information

is still correct. 

All sections in FinalForms must be complete before participating in conditioning, tryouts, practice and competition.
