This is the self-screening health form required for all staff and students entering the building. Please fill one out each morning for your child with their temperature documented. Your child cannot exit the car without the form filled out completely. Be sure to put your teacher's name on as well. We do not have a screening station at school, so please remember to take your child's temperature before coming to school.
