8:00 am- 12:00 pm

12:00 pm - 4:00 pm

4:00 pm - 8:00 pm

Please fill out these forms and bring them the hospital orientation:

Remember you will need to get your TB test completed (2-part test) before you can begin your internship AND the dress code applies to hospital Orientation.

Inters need the following Vaccination/Immunization information (which school should have). A TB test which will need to be obtained outside of the hospital (King Soopers Little Clinics provide this testing, as would their PCP).

· Proof of the following immunity or date of immunization:

· MMR (measles/mumps/rubella)

· Varicella (chickenpox)

· Tdap

· Hepatitis B

· Results of a TB Test (within past year) – you can choose one of the following methods of testing:

· Skin Test: a Two-step process. Second test to be performed 2-4 weeks or more after the first test.

· One time blood draw: choose either a Quantiferon Gold, or T-Spot (your MD or clinic will know about these) with this type, no additional testing is necessary but it is a bit more pricey

· Also need a photo of student (from waist up) for ID Badge
