BILLING

There are 2 ways your patients can be billed. By you, or by us.

IF YOU BILL

If the surgeon's preference is to bill on behalf of Dr Kollias, we are happy to accommodate this. In fact, this is the most common arrangement.

If Dr Kollias has not assisted this surgeon before, we will need to generate a new, unique Medicare provider number for your practice to use. Due to the constraints in place by Medicare, it's important that the provider number be generated online no later than the date that the surgery is scheduled to occur. In some instances, it can be backdated, but this can sometimes be difficult to do, and so we would prefer to avoid this situation if possible.

STEP 1

Download the hw019-190828 form (Application for a Medicare provider number and, or prescriber number for a medical practitioner form) at https://drive.google.com/a/iamhercules.com/file/d/11_pGGKVc_WYeZm6UEBb1KXj0SvWXL5JW/view?usp=sharing

STEP 2

The downloaded form needs to be partially completed by you. You may choose to do this electronically or print the form. Please answer the following questions on the form.

- - Question 21. Practice information
  - Question 23. Specifically your practice's ABN, ACN, Entity name & Trading As name
  - Question 24. Business type
  - Question 25. Premises type
  - Question 26. Medicare Online
  - Question 27. Medicare Easyclaim
  - Question 28. Government funded Aboriginal & Torres Strait Islander Health Service
  - Question 29. Bank account details for your practice

After you have answered these questions, please email your partially completed form to intheloop@iamhercules.com. We can then have the Medicare Provider Number created within 2 hours.

STEP 3

You will receive an email from Telly Sfyridis (Bookings Manager), containing the new Medicare Provider Number for your practice and Dr Kollias' bank account information.

IF WE BILL

As a part of our service, we're happy to bill on your behalf, if you like. We bill at the standard surgical assisting rate.
