WHAT YOU MUST COMPLETE

NOTE: all must be completed and approved before the coach can attend a practice or contest. Once we have everything completed and on file, we will notify you so you can begin your coaching duties.

1. Copy of your driver's license and social security card and high school diploma and/or college transcript

   1. For a HS coaching position, if you do not have at least 64 college credits, you are required to complete the NFHS courses listed at the end of this document.

2. Physical with TB at risk assessment 

   1. You MUST download and use this form; click LINK for form 

   2. We will need the original form signed by the doctor. You will also need to have a TB risk assessment completed by your doctor. The doctor will give you the TB paperwork once they complete the risk assessment. You will bring the Medical Examination form and the TB assessment results to the central office once complete. Both the physical and TB risk assessment may be completed at Baptist Health Occupational Medicine in Madisonville. I do recommend calling to schedule an appointment.

3. Drug Test

   1. You must complete a Drug Test at Baptist Health Occupational Medicine located at 200 Clinic Drive Madisonville, KY 42431. The phone number is 270.825.7351. Drug tests do not require an appointment unless you plan to do the physical and TB risk assessment at BHM, also.  We will have access to the results online after you complete. 

4. Child Neglect and Abuse (CA/N) report 

   1. You are required to get a background check, Child Neglect and Abuse (CA/N) report.  Below are the instructions for completing the CA/N background check for Hopkins County Schools. If you recently completed a CA/N check, you should be able to login and print and/or email the completed document.

      1. Click on this LINK to begin 

• You will create an account if you do not have one.

• Once you have created your account and verified it, you will log in.

• Select CAN tile and verification.

• Select DPP 156 from the forms tab at the top of the page.

• Select Public school employee.

• For the ENTITY REQUESTING INFORMATION section

  • Enter the board address, Hopkins Co Board of Ed, 320 S Seminary St, Madisonville, 42431 and check the box at the bottom authorizing them to share the results with  cindy.barnhill@hopkins.kyschools.us  

• Upload your driver's license.

• After you have entered all information and hit save, it should take you to a page where you will enter your payment information. The fee is $10.00.

• You should receive an email once the report has completed. This could take anywhere from 3-10 days. Once you have gotten the report back, open it and save it in a PDF format, and email it to me, please.

  • Log in and in the top left, click on DASHBOARD

  • In the bottom right, click PRINT

  • In the top right, click VIEW REPORT

  • In the top middle, click the disk and then save as PDF

  • Email me the PDF

5. SafeSchoolsTrainings: https://hopkins-ky.safeschools.com/login 

   1. SafeSchools Login: firstname.lastname

   2. There is no password for SafeSchools.

   3. We will monitor completion of them online.

   4. NOTE: If you are wanting to work with teams prior to July 1, you will have to complete the current safe schools videos AND will have to complete them again after July 1 for the year. 

6. Talent Ed records

   1. You will need to log into talented records to complete some online hiring paperwork. Go to the following link and use the username/password listed below:

      1. https://hopkinsky.tedk12.com/records

      2. Username (full email address used when you applied)

      3. Password:  Hopkins123! You will be prompted to change your password once you log in. 

7. Coaching Requirements (high school and middle school coaches)

   1. CPR/AED/First Aid; reimbursed/paid for by school; send your school athletic administrator the invoice for reimbursement.

      2. If you have a current certification, you may send a copy

      3. NOTE: your certification must be current at all times. If it expires, you are ineligible for any coaching duties.  

   2. KHSAA Sports Safety Course (KHSAA Video Tutorial)

1. HS coach 

   1. 1. 1. 1. will sign in SPORTS SAFETY LINK  You will need to log in

               1. Click the link under Current Eligibilities

               2. Scroll down to the bottom to the bottom and you will see KMS/KHSAA Sports Safety Module 1.  You will need to complete all 8 Modules.

2. MS coach 

   1. 1. 1. 1. will click on the MIDDLE SCHOOL at https://khsaa.org/safety_course/ button and send certificate to AD and Mr. Bell (matt.bell@hopkins.kyschools.us)

3. NOTE: your certification must be current at all times. If it expires, you are ineligible for any coaching duties.  

7. 3. NFHS Fundamentals of Coaching  Reimbursed by school; send your school athletic administrator the invoice for reimbursement. 

      1. NOTE: this is for high school coaches only.  It is a one time course.  If you have completed it from a prior school system, it will carry over.  

   4. High School Coaches Only

      1. If you have less than 64 college hours, you must complete the following at your own expense www.nfhslearn.com. 

         1. Engaging Effectively with Parents

         2. Teaching and Modeling Behavior

         3. Teaching Sports Skills OR a sport specific course 

   5. KHSAA annual rules clinic for sport coaching (open during season)

8. FINAL STEP: Kentucky State Police/FBI Background Check:

1. The HR department will reach out to you to schedule an appointment once we have all the above information. 

Once you have all requirements and paperwork complete, you will receive notification that you may begin performing coaching duties.  You are not permitted to be on the field with students until you receive the notification that you are cleared.
