For the 2023-2024 school year, the following circumstances will require proof of residency documentation:
Families of all new students
Families needing their address changed in Skyward
Families whose residency is not able to be electronically verified *
*If the District is unable to verify your address, you will receive a letter from the District. The letter will provide details regarding the additional information you will need to provide in order for your child to start the 2023-24 school year.
Families can email proof of residency to registration@harlem122.org or drop them off at the Harlem Administration Center.
Request a Skyward Family Access account here. If you forgot your login information for Skyward, select "forgot your login/password" on the Skyward login page.
You can translate Skyward into over 100 languages using the translation tools built into the Chrome browser. To watch a video on how to translate Skyward, click the appropriate link below:

1. On a desktop computer, click the Google Translate icon in the top web address bar. If you don't see the Google Translate icon, right click and select "Translate to English."

Verification documents from Category A and Category B must state the name of the District Resident
The service address and mailing address must match
No hand-written letters will be accepted as Legal Rental Agreements or Affidavits (see board approved forms)

If a utility bill is used to satisfy Category A, the same bill cannot be used to satisfy Category B. No hand-written letters will
be accepted as Legal Rental Agreements or Affidavits. Board approved forms must be used.)

Letters of Residence from Landlord in Lieu of Lease Affidavit 7:60-AP-2-E-1 English Español
Required if a parent or guardian seeking to enroll a child needs evidence of residency when a signed lease is unavailable. This includes an expired lease that has automatically become a month to month tenancy. Other documents will also be required to establish residency.
Top portion to be completed by parent or legal guardian and bottom portion to be completed by Landlord
Return this Affidavit along with proof of prior month payment and a document from Category B

Letters of Residence Affidavit 7:60-AP-2-E-2 English Español
Required if a parent or guardian seeking to enroll a child is living with a District Resident, such as friends or relatives, and they cannot provide sufficient proof of residency in their own name.
To be completed by the parent or legal guardian and the District Resident
District Resident is required to provide a minimum of two (2) verification documents required to establish residency
Parent or legal guardian will be required to provide a minimum of one (1) verification document from Category B (see Residency Documents for Address Changes below)
The School District reserves the right to evaluate the evidence presented, and merely presenting the items listed in this procedure does not guarantee admission
Original Birth Certificate, Certified Copy or Birth Certificate or Visa when applicable
A certified Birth Certificate is issued from the county office, in the county where the child was born. An official Birth Certificate will be marked with a county stamp or crimped seal. A certificate from the hospital (including certificate with foot prints) will NOT be accepted.
Copies will not be accepted. Our staff will make copies for school records
