Transcript Requests
Students and alumni of HSD122 may request transcripts, test scores, immunization records, etc. by completing a Transcript Request Form. The forms are in Adobe Acrobat format. You must have the free Adobe Reader, version 6.0 or higher, loaded to properly view them.
Due to federal privacy laws, a written or form request with signature is required. E-mail and phone requests for transcripts will not be accepted. Please see the request form for further instructions. This service is free for current students and graduates.

Online Transcript Requests
Harlem's current seniors and past graduates now have the option to request to send a copy of their transcript to a college or university through a company called Parchment. Please select the link below to request a transcript. This service is free for current students and $4.15 for graduates.
