** ATHLETIC PHYSICALS **
All students wishing to participate in athletics at Memorial Junior School are required to obtain a physical by their healthcare provider. The physical examination must take place within 365 days of the first practice.
Step 1: Obtain a physical
A current/valid physical is one that has been performed by a healthcare provider and has been cleared by the school physician before a student can participate/tryout. The History Form and Physical Examination Form must be completed by a health care provider and do not need to be submitted to the nurse's office. Both forms should be kept by the healthcare provider who completes them. ONLY THE COMPLETED MEDICAL ELIGIBILITY FORM SHOULD BE SUBMITTED TO THE SCHOOL. The school physician will review the Medical Eligibility Form upon receipt. The school district will then provide the parent with written notification signed by the school physician stating that participation in school athletics is approved or not approved.

It is recommended that your own physician conduct the medical examination. If you do not have a primary care/pediatrician or cannot be seen by your primary care physician, you may contact our school doctor, Dr. Michael Kelly, DO at 973-736-1939 to make an appointment at his office for the physical.
Step 2: Register for Sports
Parent/Guardian and student-athlete must complete the Arbiter Sports (formerly known as FamilyID) registration process. (Registration for fall sports will begin on 7/22/2024)

If your student-athlete has Asthma he/she will not be eligible to participate in sports until a signed Asthma Treatment Plan has been received from your private physician. If your student-athlete has been symptom free for an extended period of time and does not require the use of an inhaler, a physician statement indicating that an inhaler is no longer necessary must be submitted.
If your student-athlete has a life-threatening allergy that requires an Epi-Pen he/she will not be eligible to participate in sports until the signed administration of epinephrine forms is received from your private physician.

Arbiter Registration (Family ID) Registration Steps:
1. Click on the blue ("CREATE ACCOUNT) in the top right-hand corner.
2. Create your secure Arbiter Registration (formerly Family ID) Account by entering your name, email address twice, a password twice, and checking the "I agree to the Arbiter Registration terms of service" box. Then select the green "Create Account" button.
3. You will receive an email with a link to activate your new account. (If you do not see the email, check your spam or junk folders)
4. Click the link to activate your account.
5. This will bring you to the Arbiter Registration (Family ID) dashboard.
PLEASE NOTE THAT YOU MUST USE THE LINK THAT SAYS "MEMORIAL JUNIOR SCHOOL" TO REGISTER FOR A SPORT.
1. After creating an Arbiter Registration (Family ID) account following the email verification link or logging on as an existing user, you will be brought to your dashboard. Please type Memorial Junior School in the search box.
2. Find the Memorial Junior School Athletic Program and select the blue link with the programs title to head to the athletic registration page.
3. The programs landing page will show a description of the program with a green "REGISTER NOW" button or simply scroll down the page until you see the blue header that says "SELECTIONS" to begin your registration process.
4. At any time, you may login to www.familyid.com to update your information and check your registration.
