Athletic Clearance

Online Clearance Portion

Online clearance must be completed for each sport you intend to participate in this year.

Step 1

Go to athleticclearance.com - CLICK HERE

Step 2

Complete the Log-in Process.

Step 3

Complete the Online Forms.

Step 4

Complete the Online Signature Agreements.

Step 5

Print and Sign the Confirmation/Consent Form. This form will be emailed to your when your online athletic clearance have been approved.

Step 6

Bring a COPY of your current physical, the signed clearance/consent email, and signed COVID Waiver to Ms. Schow in the VHS Finance Office. You will then be given a clearance ticket to give to your coach at tryouts. 

*ALL students participating in athletics MUST have medical insurance coverage valid for the entire athletic season

** - If needed, CLICK HERE to purchase Student Insurance. After insurance is purchased, update your account with the needed insurance information.

Physical Form Portion

Physicals are required annually and must be valid throughout the entire season of your sport.

Step 1

Download the Physical Form - CLICK HERE

Step 2

Print name and DOB on the Physical Form prior to doctor visit.

Step 3

Have a licensed physician complete the bottom of the Physical Form.

Step 4

Make a COPY Physical Form for each sport you intend to play this year.

Step 5

Bring a COPY of your current physical, the signed clearance/consent email to Ms. Schow in the VHS Finance Office. You will then be given a clearance ticket to give to your coach at tryouts. 

*Keep your original physical form for your own records and to be used for other sports during the school year.
