Welcome to Green Primary and Elementary!

Before you get started, please see the following information.

For preschool, your child must be 4 years old on or before September 30, 2022 and potty-trained.
For kindergarten, your child must be 5 years old on or before September 30, 2022.
If your child is currently enrolled at Green Preschool, you DO NOT have to enroll your child again. Please email jessica.mcilhenny@greenbobcats.org and state that you want to continue enrollment for kindergarten at Green.
Only a LEGAL guardian may enroll a child.
If you do not live in Green Local School District you may still apply for enrollment. You will need to email jessica.mcilhenny@greenbobcats.org for further information.
If you are a foster parent and wish to enroll your foster child, please email jessica.mcilhenny@greenbobcats.org for further instructions.
Our preschool program spots are filled based upon qualifying criteria first; then date of registration. Date of registration is the date both Final Forms has been completed AND all required documents have been submitted. It is extremely important that you submit ALL required documents when you register. Only applicants with completed Final Forms AND all required documents will be considered for the PS program. Your child will NOT be considered for our PS program or be put on our waitlist until we receive ALL of the required documents. The final roster for preschool will be determined later in the summer. Kindergarten classroom assignments will be determined in the fall. Further information will be provided at a later date.

STEP 1

Complete Final Forms

You will need a personal email in order to create a Final Forms account. If you do not have an email, you will need to create one first. The LEGAL guardian must have the Final Forms account. NOTE: Final Forms will state that you need to make an appointment. You do not need to make an appointment. Step 2 will satisfy the appointment.

Click on the link, Final Forms, create a parent/guardian account. **If you already have a Final Forms account because of other children enrolled at Green, you do not need another account. You may add your new student to your existing account.
From your parent/guardian account, register your child. Please make sure you are registering your child for the 2022-2023 school year.
You will see a list of forms that are shaded red. As you complete each form, they will turn green. Once they are all green, you have completed Final Forms.
Proceed to Step 2. Completing Final Forms does NOT finish your child's registration application.

STEP 2

Submit Documents

Please note: Your child will not be considered for a PS spot until ALL correct documents are submitted.

birth certificate (official copy, not hospital copy)

immunization record

social security card

parent/guardian ID

proof of residence (current utility bill, lease/purchase agreement)

custody documents (if applicable)

income info (PRESCHOOL ONLY - W-2 or 2 most recent pay stubs)

How to submit? Email, Mail, Fax, or Upload

jessica.mcilhenny@greenbobcats.org You may email clear screenshots of the documents or if you have the notes app you may scan and email. In the notes open a new note, press the camera button, select scan. Once you scan the document you may email it easier than a photo.

Upload documents in your child's Final Forms account. If you do this, you MUST email jessica.mcilhenny@greenbobcats.org. She does not receive a notice from Final Forms that you have uploaded.
