DELEGATE APPLICATION FORMS
Anyone in grade 9 - 12 is welcome to attend D.A.R.E! Applications will be accepted on a first-come, first-serve basis, so don't wait! We only have 120 spots!
There are 3 things to complete in order to be given a spot:
1. Fill out your permission form & have your guardian sign it. Permission forms are linked below for you to print, or we have a few copies in the foyer. Return it to Ms. Zimmer (in the front foyer Monday - Friday or in room 121).
2. Pay your $90 fee on School Cash Online. Please come to the foyer if you need to pay cash (online is preferred).
3. Complete the Google Form to give information for our D.A.R.E. weekend!

It is important to note that there are NO in/out privileges. You need to commit for the entirety of the retreat. There will be no refunds available.
