School Physicals: New York State requires all 9th & 11th grade students and new entrant students to the School District to have a physical on file with the school. If your child has had his/her physical, but has not turned in the physician's form, please do so as soon as possible.
*It must be on the Required NYS School Health Examination Form.*
Students who have not turned in documentation of a physical will be examined by the school physician.

Grade 9: Scoliosis (Boys only) & Grade 11: Distance & Near Vision Screening & Hearing Screening.
Immunizations:
All students must be immunized by NYS regulations. For more information, please visit www.schoolhealthservicesny.com.
2025-26 School Year NYS Immunization Requirements for School Entrance/Attendance
*Students entering 12th grade REQUIRE (by NYS) a “Meningitis A” (or MCV4) vaccine after the age of 16.
If a Doctor or Specialist requests a student not participate in physical education or sports, the nurse must receive a clearance note from that doctor/specialist before they will be allowed to return to gym or sports.
Concussions typically require a Return to Play (RTP) sometimes with Physical Therapist with clearance from ortho doctor or a concussion specialist .
If a student was not cleared to play PE when the school year ended in June, the nurse needs the clearance from the Physician to allow student to play or an updated medical note to keep student out in September.
For injuries: Please have the doctor write for any accommodations your child may need to be in school, such as: crutches, brace, 5-min. early pass, book buddy, and/or elevator pass, etc.
If a student requires any medications (Epi-pen, Benadryl, Albuterol Inhaler), even Over-The-Counter medications (Advil/Motrin (Ibuprofen), Tylenol...etc.) the nurse needs a prescription from the doctor, otherwise a parent/guardian may come to administer medication. The prescription must be on school form, sign by the doctor and parent/guardian. Under NYS Nurse’s Laws, the Nurse may not administer any medications without a prescription. As always, please contact the Nurses office with any questions or concerns. 🙂
If a student has diabetes or seizure disorder, please notify your nurse for the necessary annual paperwork for Diabetes and Seizure / Epilepsy.
Sport Clearances
All sports will be cleared through Final Forms (www.FinalForms.com) only. Access to Final forms opens 30 days prior to the start of your sport. *Do not give paperwork to your Coach, Secretaries or Administrators. Do not mail it. You will receive an email when your Child / Athlete is cleared through final forms.* Only upload physicals to final forms for sports. Please remember to upload orders for ANY medications and MD clearances you need for clearance. All medical history including injuries & allergies must be documented on Final Forms. You will not be cleared without completed forms and accurate medical history. Please Do Not wait until the last minute to try and get cleared, the nurses Will Not be able to accommodate last minute clearances. SACHEM EAST HS Boys and Girls Sports Physicals will be on Wednesday, May 28th, 2025 @ 2:30 pm. Health history (pages 3, 4, 5) must be completed and signed by Parent / Guardian prior to the Physical.

Sports Physicals will also be offered for Sachem athletes at North HS on 5/29, at Sagamore Middle School on 6/2, at Samoset Middle School on 6/3, and at Seneca Middle School on 6/4/2025.
