Exposures
Please notify us of any exposures via This Form. Exposures include, but are not limited to:
- Contact of a patient’s droplets/body fluids with a provider’s eyes, mouth, nose or open skin;
- Needle-stick injuries;
- Failure, tear of PPE w/ fluid contact
Typically, the use of proper PPE will not result in a reportable exposure. However, report ANYTHING of concern so that we can review and ensure your safety.
