Three plans to choose from:
Aetna Health Saver
Aetna Select
Aetna Advantage
Medical Plan Comparison Chart 2027
All three plans provide comprehensive medical coverage and rely on national networks of providers for the care you need.
ALEX at start.myalex.com/ghco can estimate your total yearly cost for each plan, including your deductible status and the 2027 Garner incentive.
Two plans to choose from:
Aetna Dental Value
A lower-cost comprehensive plan that limits orthodontia coverage to children through age 19.
Aetna Dental Plus
A comprehensive dental plan that provides a higher level of coverage and orthodontia for covered members, regardless of age.
Two cleanings a year are covered at 100% with a covered provider.
Two plans to choose from:
EyeMed Vision Value
This plan has a lower monthly premium, but you will generally pay more out-of-pocket for your exams and eyewear.
EyeMed Vision Plus
This plan has a higher monthly premium, meaning you’ll pay more out of your paycheck. However, this plan may come with lower out-of-pocket costs when you use vision services.
Your plan covers an annual eye exam, whether or not your prescription has changed.
If you enroll in the Aetna Health Saver, you can choose to contribute to a Health Savings Account (HSA). The money you set aside can be used for eligible health care expenses incurred during a plan year or in future years, and you can earn tax-free interest and investment returns.
Contribute pre-tax up to:
$4,500/employee only
$9,000/family
Additional $1,000 age 55 and over
Plus, the company will contribute to your HSA that you establish with Inspira Financial* up to:
$400/employee only
$600/family
*The Inspira Financial HSA is not maintained or sponsored by the Company and is not subject to ERISA.
The company contribution is only available if you elect the Inspira Financial HSA through your benefits enrollment. You may open an HSA with any financial institution, but you must elect Inspira Financial to receive the company contribution. Even a $0 election in Workday ensures you receive the company contribution — but you have to make the election, and it doesn't carry over from last year.
The money you set aside can be used to pay for eligible expenses (defined by the IRS) expected to be incurred during a plan year.
The Health Care Spending Account (HCSA) is available to benefit eligible employees, but not those enrolled in the Aetna Health Saver medical plan option. You don't have to be enrolled in any of the company's medical plans to participate in the HCSA. The HCSA allows you to set aside pre-tax money for reimbursement of eligible medical, dental, and vision care expenses.
Health Care Spending Account
Contribute pre-tax up to $3,400/year.
A Dependent Day Care Spending Account (DCSA) lets you save money on day care expenses for your dependents. The DCSA reimburses eligible dependent day care expenses so that you (and your spouse, if applicable) can work. It is not for health care expenses.
Dependent Day Care Spending Account
Contribute pre-tax up to $7,500/year. Highly Compensated Employees (defined as those with annual earnings of $150,000 or more) annual contribution limit is capped at $6,000.
HCSA and DCSA balances may be used for expenses incurred through March 15 of the year following the year the contributions were made. Inspira Financial must receive your claims by May 31 of the year following the year you made the contributions. If you don't use the money in these accounts within the designated time frame, you forfeit any remaining balance.
Garner reimbursements and Carrum coverage may lower your out-of-pocket costs, which you can consider when choosing your contribution. Any balance left after March 15, 2028, is forfeited.
Long-Term Disability (LTD) coverage enables you to receive part of your income while you are unable to work due to a total, or in some cases a partial, disability:
50% of base salary, up to $100,000
66.67% of base salary, up to $199,500
Note: At initial eligibility, you may elect any level of coverage. Proof of good health is required to subsequently increase your coverage.
When you're first eligible, you can choose either coverage level without proof of good health. If you want to increase your coverage later, you'll need to provide proof of good health.
$50,000 of Basic Employee Life is provided by the Company at no cost.
You may purchase additional insurance:
Employee Optional Life Insurance
pays a benefit to your beneficiary if you die: ½, 1, 2, 3, 4, 5, 6, 7, or 8x base salary
(Coverage maximum: $2,000,000)
Spouse Optional Life Insurance
pays a benefit to you if your dependent 1 dies: $10,000, $25,000, $50,000, $100,000, $150,000, $200,000
Child Optional Life Insurance
pays a benefit to you if your dependent 1 dies: $5,000, $10,000, or $20,000 per child
Note: If your election requires Evidence of Insurability (EOI), you will receive a task titled 'Complete Hartford EOI' in your Workday inbox shortly after you submit your elections. Please note that your coverage will remain at its current 'default' level until your EOI is reviewed and approved.
1See dependents eligible for life insurance and AD&D under Summary Plan Description (Chapter 2).
The coverage level you choose when you're first eligible doesn't need a medical review. Increases after that may require Evidence of Insurability (EOI).
Accidental Death & Dismemberment (AD&D) coverage can supplement your life insurance, providing additional coverage if you die or suffer a severe injury due to a covered accident:
½, 1, 2, 3, 4, 5, 6, 7, or 8x base salary
(Coverage maximum: $2,000,000)
Coverage is also available for your family 1 based on family composition:
Spouse Only: 60% of Employee Amount
Child Only: 20% of Employee Amount
Family:
Spouse: 50% of Employee Amount
Child: 15% of Employee Amount
Maximums:
Spouse: $1,000,000
Child: $150,000

Business Travel Accident coverage is already yours at no cost. You can review and update your beneficiary designations in Workday during enrollment.
ARAG® Legal Insurance gives you access to professional legal help for everyday and other legal matters, such as:
Estate Planning. Meet with a network attorney to plan who you want to leave your assets to, name a guardian for any young children, and determine who will be in charge of your finances and medical decisions if you’re unable to make them yourself.
Consumer Protection. Get advice from a network attorney for issues with auto repairs, contractors, buying a car, or various types of fraud.
Family Law. When you face challenges like divorce, guardianship questions, or school matters, our network attorneys can provide legal guidance.
Legal coverage has a monthly premium and includes services like estate planning, consumer protection and family law guidance.
Deductibles are increasing for all three Aetna medical plans in 2027. The deductible is the amount you pay for covered services before your plan begins sharing the cost.
But we’re raising the Garner incentive from $1,000 to $2,000 for individuals, and $2,000 to $4,000 for families to keep pace. If you use Garner to see Top Providers, your reimbursements can help offset some or all of the deductible increase.*
*Aetna Health Saver members must meet the IRS minimum deductible ($1,750 for individuals and $3,500 for anyone covering dependents in 2027) before any costs can be reimbursed by Garner or paid for by Carrum Health.
What this means to you
You'll pay a bit more before your plan starts sharing costs. But Garner pays you back when you see the doctors it rates highest. If you use Garner to see Top Providers, your reimbursements can help offset some or all of the deductible increase.
What you can do
Activate your free Garner account at garner.guide/access-garner any time.
Already using Garner? Nothing to re-enroll. The higher 2027 incentive applies automatically.
You can add Top Providers to your Care Team in the Garner app and check it when booking appointments. Find a Garner Top Provider
Visit those providers. Reimbursement is issued automatically based on your claims — you don't need to tell your doctor anything special.
Network providers cost less: out-of-network deductibles are also increasing, and Aetna Select is a network-only plan.
The opportunity
Garner reimbursements can help offset some or all of the deductible increase: up to $2,000 individual / $4,000 family back in 2027.
The out-of-pocket maximums across most medical plans will increase in 2027. The out-of-pocket maximum is the most you'll pay for covered, network care during the plan year.
What this means to you
The out-of-pocket maximum caps what you'd pay for covered, network care in a year with higher medical costs.
What you can do
When comparing plans, it can help to look at out-of-pocket maximums along with premiums. Advantage has the lowest maximum. Health Saver has the lowest paycheck cost and includes a company HSA contribution.
ALEX at start.myalex.com/ghco can estimate your out-of-pocket costs for each plan.
Network providers cost less: out-of-network maximums are double the network amounts.
The opportunity
Garner reimbursements and Carrum's covered procedures may help lower your out-of-pocket costs.
Beginning in 2027, Aetna plan members will have access to Carrum Health, which connects you with specialized providers and facilities for eligible major, pre-scheduled surgeries, including joint replacement, neck, back, and heart surgery, and cancer care, through Centers of Excellence (COE).
Carrum coordinates your care, and all medical costs associated with eligible procedures and treatment options are available to you with no out-of-pocket cost*. It’s that simple.
Note that Carrum Health services are limited to pre-scheduled, non-emergency care, and that examinations, tests, prescriptions or other medical services that may be required before or after your visit to the Carrum Center of Excellence, along with any medical services not performed by a participating Center of Excellence facility or physician, including necessary pre- and post-acute care, may not be covered under the Carrum Health benefit and may therefore be subject to the coverage limits, cost sharing, and other terms of the Plan.
Visit GHConnect for additional details, or to learn more about Carrum Health or request a consultation with a Center of Excellence (available January 1, 2027):
Download the Carrum Health app, available on both iPhone and Android devices or online by visiting carrum.me/COEbundles, or
Call Carrum Health at 888-450-7345, Monday-Friday 9 am-8 pm ET
Participation in the Carrum COE program is voluntary for most procedures; a Covered Person may instead elect a non-Carrum provider, in which case the Plan's standard cost-sharing and network terms apply. Amounts paid by Carrum shall not count toward the Covered Person's deductible or out-of-pocket maximum, as no cost-sharing amount is actually incurred. In addition, services received through the Carrum COE program are not eligible for any Garner HRA incentive or reimbursement.
Participation in the Carrum COE program is required for coverage of bariatric surgery. Coverage through Aetna's Institutes of Quality (IOQ) program is no longer available. Bariatric surgery services obtained from providers outside the Carrum COE network are not covered under the Plan.
*Aetna Health Saver members must meet the IRS minimum deductible ($1,750 for individuals and $3,500 for anyone covering dependents in 2027) before any costs can be reimbursed by Garner or paid for by Carrum Health.
What this means to you
If you need surgery or cancer care, there's now a way to get care from a Carrum specialist at no cost to you*. Carrum finds the doctor, books the care, and handles the bill.
What you can do
If you or a covered family member is facing a procedure, such as orthopedic or spinal surgery, cardiac care or cancer treatment, contact Carrum to find out whether it's eligible:
Register at carrumhealth.com/members starting January 1, 2027.
Download the Carrum Health app, available on both iPhone and Android devices or online by visiting carrum.me/COEbundles, or
Call Carrum Health at 888-450-7345, Monday-Friday 9 am-8 pm ET
Not sure whether Carrum Health or Garner is the right choice for you? Call Ask Benefits at 855-424-0400.
The opportunity
$0 out of pocket* for a covered procedure — instead of your full deductible and coinsurance, with the search for the right specialist and facility done for you.
*Aetna Health Saver members must meet the IRS minimum deductible ($1,750 for individuals and $3,500 for anyone covering dependents in 2027) before any costs can be reimbursed by Garner or paid for by Carrum Health.
Cost shares for brand-name drugs are going up across all plans — driven by rising pharmacy costs nationwide. Generic drug cost shares are staying the same.
What this means to you
Brand-name prescriptions will cost you more in 2027. Generic cost shares aren't changing, so if you take a generic, your cost share stays the same.
What you can do
Ask your prescriber or pharmacist whether a generic is available. Generic cost shares aren't changing in 2027.
Check CVS Caremark's formulary tools for lower-cost alternatives before you fill — 800-552-8159 or caremark.com.
Use mail order for maintenance medications you take every month — a lower per-unit cost.
The opportunity
Generics cost you no more in 2027 than they did in 2026. If a generic is available and right for you, it may cost less than the brand-name version. And keep in mind that eligible prescriptions from your Garner provider may also be reimbursed up to the limit.
We recognize weight loss is a real and meaningful health goal, and we don’t make this decision lightly. The rapid and dramatic rise in the cost of weight loss medications has reached a point where continuing to cover them would affect premium costs for everyone.
Effective January 1, 2027, FDA-approved prescription drugs for weight loss and weight management are excluded from coverage. This exclusion also applies to any additional approved uses of medications that were originally FDA approved to treat obesity.
GLP-1s FDA approved for diabetes care are not affected. If you're impacted, options may include manufacturer savings programs and/or HSA/FSA funds.
What this means to you
If you take a prescription drug only to lose weight, the plan will stop paying for it on January 1. If you take a GLP-1 for diabetes, nothing changes for you.
What you can do
If you're taking one of these medications, here are some options to explore before January 1, including talking with your prescriber:
Manufacturer savings programs. Some manufacturers offer direct-to-patient pricing — check the manufacturer's website for current offers.
HSA and FSA funds. You may be able to use pre-tax balances toward out-of-pocket costs, depending on the clinical indication. Consult a tax advisor.
Garner Health. Find Top Providers who specialize in comprehensive weight management — care that stays covered.
The opportunity
Comprehensive weight management from a Garner Top Provider is still fully covered — and still earns your Garner reimbursement.
Note: Garner reimbursements do not apply to weight loss drug costs, since they are no longer a covered plan expense.
CVS announced that beginning January 1, 2027, no cost/low cost MinuteClinic visits will no longer be available. Visits will instead be covered like other network visits under your medical plan — subject to your deductible and cost sharing.
What this means to you
A MinuteClinic visit used to be free, or at a reduced cost. Starting January 1 it's billed like any other network doctor visit, so it counts toward your deductible and cost sharing.
What you can do
If you don't have a primary care provider, Garner can help you find top-rated doctors in your network at garner.guide/access-garner.
Teladoc 24/7 and the Aetna Nurse Line are also part of your benefits.
The opportunity
Garner can help you find a new doctor and reimburse you when you see them. Preventive care visits are still covered at 100% when you see a network provider.
For Aetna Health Saver members, the amount you can contribute to your Health Savings Account (HSA) is increasing for 2027, per IRS guidelines. You will be able to contribute up to $4,500 if you only cover yourself and $9,000 if you cover your family. (If you’re 55 or older in 2027, you can contribute an additional $1,000 as a catch-up contribution.)
Remember: the company also contributes to the HSA you establish with Inspira Financial: up to $400 employee only / $600 family.
What this means to you
You can set aside a little more pre-tax money in 2027. Garner reimbursements and Carrum coverage may lower your out-of-pocket costs. You can factor that in when you decide how much to contribute.
What you can do
Re-elect every year. Health Savings Account (HSA) elections do not carry over.
Don't double-dip. Do not use HSA funds for costs that Garner will reimburse — the IRS does not permit it.
Use ALEX at start.myalex.com/ghco, or chat with EVA, our new AI assistant in GHConnect.
The opportunity
Every dollar you set aside goes in pre-tax — plus up to $600 from the company with an HSA with Inspira1.
Note: The annual company HSA contribution is divided by your total pay periods, and this amount is contributed to your Inspira Financial HSA with each pay that you are eligible for and enrolled in the HSA. While you may have an HSA through any provider you choose, Aetna Health Saver Plan members must be enrolled in the Inspira Financial HSA to receive these company contributions.
1 Your HSA with Inspira Financial® is an individual trust or custodial account that you establish with Inspira Financial® if you meet the eligibility requirements in Internal Revenue Code Section 223. The HSA that you establish with Inspira Financial® is not sponsored or maintained by GHC or any of its affiliates.
We offer two Flexible Spending Accounts (FSAs). Here's what's happening with each for 2027.
Health Care Spending Account (HCSA): The maximum you can contribute is going up to $3,400. The HCSA is available to benefit eligible employees, but not those enrolled in the Aetna Health Saver medical plan option. You don't have to be enrolled in any of the company's medical plans to participate in the HCSA. The HCSA allows you to set aside pre-tax money for reimbursement of eligible medical, dental, and vision care expenses.
Dependent Day Care Spending Account (DCSA): The maximum stays at $7,500 for 2027. Use pre-tax dollars to pay for eligible day care for your dependents (not health care expenses). The limit will be $6,000 for those earning $150,000 or more.
What this means to you
You can set aside up to $3,400 pre-tax for eligible health care costs and up to $7,500 for dependent day care in 2027. The money comes out of your paycheck before taxes, so you keep more of what you earn. Garner reimbursements and Carrum coverage may lower your out-of-pocket costs. You can factor that in when you decide how much to contribute to your HCSA.
What you can do
Start with what you know. Look at what you spent this year on costs you can predict, like prescriptions, glasses or contacts, dental work, and regular copays. Use that to consider your HCSA amount.
Re-elect every year. Your HCSA and DCSA elections don't carry over.
Don't double-dip. Don't use HCSA funds for costs that Garner will reimburse. The IRS doesn't allow it.
Watch the FSA deadline. Use HCSA and DCSA funds by March 15, 2028; any remaining balance is forfeited.
Use ALEX at start.myalex.com/ghco, or chat with EVA, our new AI assistant in GHConnect.
The opportunity
FSAs work best for costs you know are coming. A careful estimate gets you the tax savings without the risk of losing money at the end of the year.
Three programs. One goal. Support when you need it.
The following medical extras are available to Aetna plan members: Garner, Carrum, and 2nd.MD. Each helps in a different way. Here's when each one can help.

Everyday care — finding a primary care provider, a specialist, or any network doctor.
Use it for
A major procedure — orthopedic or spinal surgery, cardiac care, cancer treatment.
Use it for
A second opinion after a major diagnosis or treatment decision.
