Team Registration Payment link (via Credit Card) is below. Team Registration cost is $325 per team. Teams may request an invoice if needed to process payments. Payment must be received by 5PM on Monday, March 2nd.

ORIGINAL DOCUMENTS must be brought for the following: Team Tournament Registration Form, Team Player Certification Form, and Unified Athlete Certification Form (If applicable)
Team Registration Cost: $325 - If registering multiple teams, payment will be submitted separately for each team. Payment must be received by March 2nd, 2026 at 5:00PM.
Tournament registration form must be completed by all teams. Original document MUST be brought to the tournament and turned in at team registration.
Player certification form must be completed by all teams. Original document WITH Registrar's stamp MUST be brought to the tournament and turned in at team registration.
Team Code of Conduct form must be completed by all teams. A scanned copy of this document is acceptable. We need a scanned copy or original on file prior to play.
Individual Waiver Form must be completed by all participants. A scanned copy of this document is acceptable. We need a signed waiver, either scanned or original, from each participant prior to play.
Unified Athlete Certification Form must be completed for Unified Teams. Original document with approved signatures MUST be brought to the tournament and turned in a team registration.
