NON-ACADEMIC AWARDS
General Guidelines and Instructions
To promote and recognize excellence in student leadership, community involvement, and apostolic commitment, the Student Services Center provides Non-Academic Awards to qualified students or student organizations. The following guidelines are issued to ensure a fair, consistent, and meaningful awards process.
I. General Policies
One Award Category per Applicant
Students or student organizations may apply for only one award category. Multiple applications are strongly discouraged unless the applicant clearly meets the qualifications and demonstrates merit in more than one category.Endorsements Required
All applications must be:Endorsed by the organization’s moderator
Noted by the College Dean

II. Application Form Components
Guidelines Compliance Section
To be considered for an award, applicants must strictly follow and satisfy all stipulated guidelines.Indicate the appendix label corresponding to each guideline.
Obtain certifications from the following offices, as applicable:
Registrar: for academic residency, subject load, and grades
Prefect of Student Formation: for conduct and disciplinary records
Student Activity Coordinator: for leadership roles and involvement in recognized student activities
Criteria and Supporting Evidence
Clearly provide all required information. Attach labeled appendices (certifications, documents, and proof of involvement) to validate each claim.Applicant’s Signature
The applicant must sign the form to attest to the accuracy and truthfulness of all submitted information.Committee Rating Section
This section is reserved exclusively for the Awards Committee and should not be filled in by the applicant
III. Award Application Pack Requirements
Applicants must compile and submit a complete award application pack, which includes:
Accomplished Application Form (serves as the cover page)
Endorsement Letter from the moderator, noted by the Dean
Personal Essay reflecting how the applicant embodies the three CJC pillars: Excellence, Community, and Apostleship
Labeled Appendices containing required certifications and supporting documents
IV. Submission Process
Submit seven (7) soft-bound hard copies to the Student Activity Coordinator’s desk

All submissions must be received on or before the announced deadline
V. Deliberation and Interview
Applicants and/or their moderators must be available during the deliberation period. The Awards Committee may request their presence for further inquiries or clarifications.
