1. In order for student to attend the try-out, parent must fill out the Sport Try-out Form and return it to the office prior to the Try-out.

2. If student is selected to join the team, the Sport Pre-participation Physical Evaluation form needs to be completed by a physician and returned to the office prior to the first practice.

3. If parents wish to drive students (other than their own child) to the game, please submit volunteer registration online here. Once it is approved, you are allowed to drive other children.
