Dental Exam Form

Dental exams are required upon:

• School entry (Redbird or New Kindergarten Students)

• 3rd grade

• 7th grade

Dental Exams that have been performed within the year before the start of the mandated grade level can be accepted.

( i.e. For the 2020-2021 school year, dental exams must be dated after Aug 19, 2019)
