The following docuements are required:
Student’s Birth Certificate
Student’s Social Security Card
Driver’s License of enrolling parent
Proof of Residency (a current gas, water, or electric bill (within the last 30 days). If you have not yet received a bill, a lease agreement, contract of sale, or closing papers will suffice.
Immunization Records (You must provide in order to enroll) Review the website below for updates: http://www.friscoisd.org/departments/health-services/immunization-information
A current transcript must be provided for students entering 10th – 12 grade. You can obtain an unofficial transcript from the student’s previous school. Failure to provide a transcript will delay the individual meeting with a counselor. HHS will request OFFICIAL records at a later date.
A recent report card must be provided for students entering 9th grade. Include report cards from previous grades if high school credit was granted (i.e. Algebra I, Geometry or health).
Texas Residents: EOC Score Sheets for test taken Algebra 1, Biology, English I, English II & US History
Documentation from any special needs or programs, (Special Education, 504, GT, ESL, etc.).
If applicable, any legal documents regarding custody.
*HHS will not request records prior to enrollment. Official records will be requested following enrollment.
** Please note: Your child’s previous school will be able to provide you with an unofficial transcript, immunizations and a final report card.
