Special Meals forms are for students that require a special diet at school or modification of lunch tray (i.e type 1 diabetes, food allergies, conditions or medications that cause food to be omitted from diet).
If your child has been diagnosed by a healthcare professional with any of the following health concerns, you will need an Emergency Action Plan (EAP). This form is to be completed by a physician/NP and returned to the school nurse.

The school nurse will do screenings for head lice per their discretion and per request of staff or parents. If a student is found to have a live louse (bug), the child will be sent home. The child may return to school once they have been treated with a lice treatment product and no live bugs are seen. When the child returns to school, they should be accompanied by a parent/guardian. The child's hair will be rechecked by the school nurse and if they are found to be free of live bugs, the child will return to class.
