Step 3: Gather ALL Paperwork

Last report card OR copy of transcript
Birth Certificate
Immunization records with TB test
TWO proof of addresses
- Parent/guardian driver's license
- Utility bill
- Rental agreement
Current IEP or 504 must be submitted at the time of registration (If Applicable)
