Step 3: Gather ALL Paperwork

• Last report card OR copy of transcript

• Birth Certificate

• Immunization records with TB test

• TWO proof of addresses

  • Parent/guardian driver's license

  • Utility bill

  • Rental agreement

• Current IEP or 504 must be submitted at the time of registration  (If Applicable)
