This is the packet needed for a new physical. A student's physical is good for 1 calendar year and must be completed on the papers in this packet. UNIVERSAL FORMS ARE NOT ACCEPTED. Please be sure to complete the entire packet, explain any health history questions marked "yes" in the space provided, and make sure your doctor completes all signature lines. Your paperwork will be returned to you in any portion is incomplete.
Health History Update
High School/Middle School Packet (yellow packet)
This packet is for a student who already has an approved athletic physical on file and that physical is not more than 1 year old.
***If you have an Epi-pen or inhaler, your physical will not get approved if you do not have a treatment plan on file with the school nurse for the current school year.
