Attendance at care team meetings (case conferences, school meetings, stakeholder reviews) where it supports the plan and outcomes
Prompt, professional case notes: clear, strengths-based, practical, and aligned to goals
Case management support: arranging sessions/visits, coordinating logistics, and planning activities that match goals and capacity
Therapeutic reports: progress updates, recommendations, and next steps written in professional language
Matched practitioner + continuity: we aim for the same therapeutic team member to attend consistently; if coverage is needed, you’ll get the same professional calibre with clear handover
Real-world delivery: support happens in context (home, community, OOHC settings)—not just “in theory”
Collaborative approach: we work alongside carers, schools, child protection, allied health and coordinators (not in silos)

Subject to capacity, Empower YOUth can commence supports within approximately 2 weeks of referral. The initial two-week period is used to collect, review, and confirm intake documentation, establish goals, and complete practitioner matching to support safe, effective service delivery.
We allocate participants to a matched therapeutic team member (based on goals, presentation, environment, and fit). Wherever possible, we prioritise consistency — the same practitioner attending each session. If coverage is ever required (leave/illness/scheduling), support is provided by another team member of the same highly professional calibre, with clear handover and continuity.
To help us commence supports as promptly as possible after a referral is received (often within 2 weeks, subject to capacity), we follow a simple intake process:
Intake Form - You (or the participant/guardian) complete our intake form so we can confirm goals, context, key contacts, and service fit.
Medical Form - We collect relevant medical/health information to support safe, informed service delivery (including any risks, medications, and considerations we need to know).
Intake meeting (phone or in person) - We schedule an intake meeting to clarify goals, confirm priorities, discuss safety/risk considerations, and ensure we match the right therapeutic team member.
Document collection (as needed) - We then request any additional information required to commence safely and effectively (e.g., plans, reports, risk information, school/carer context), depending on the participant and referral pathway.
Service Agreement - We send a Service Agreement outlining the agreed supports/program, costs, dates, days/times, and any key terms. Services commence once the agreement is confirmed and signed.
