畢業生要上網填寫以下2份問卷 請點選回答後 才可辦理離校手續

1.畢業生離系問卷 2.畢業問卷  (學士班)  (碩士班)  (博士班)  

親愛的本學期應屆畢業生(含大學部、碩士班、博士班)：

為建立系上畢業系友資料庫及因應個資法，將不再使用紙本核對個人資料及收集問卷

請今年即將畢業的同學上網填寫畢業生離系手續單及畢業問卷。

註：
1. 本資料僅提供系上系友聯絡及教育部高等教育校務資料庫調查、系所評鑑等使用。
2. 此為本系離校手續之一，請同學務必上網填寫。
3. 碩博士班學生請另下載填寫研究生離校手續單(如附件)，供指導老師簽名及系圖繳交論文用。

Thank you for your contribution in making our department better! 

Graduating students must complete the following two online questionnaires before the school leaving procedures. 

1. Graduates’ Department Exit Questionnaire

2. Graduation Questionnaire (Undergraduate Program) (Master’s Program) (Doctoral Program)

To All graduating students of this Semester

(Including Undergraduate, Master’s, and Doctoral Programs): 

To establish the department’s alumni database and comply with the Personal Data Protection Act, paper-based personal data verification and questionnaire collection will no longer be used.

Students graduating this year are required to complete the Leaving Confirmation for Graduate students and the Graduation Questionnaire online. 

Note: 

1. The collected data will only be used for department alumni contact purposes, Ministry of Education higher education database investigations and department evaluations. 

2. This is part of the department's School Leaving Procedure; students must complete the forms online. 

3. Master’s and Doctoral students must also download and complete the Department's School Leaving Procedure Form (see attachment) with advisor signatures and submission of thesis to the departmental library.
