Physical Packets

The Ohio High School Athletic Association requires all student-athletes to have a valid physical completed before participating in athletics.  OHSAA rules allow any sports physical completed between May 1st and June 1st to count toward the entirety of the following school year. Any physical completed afterward is only acceptable for one year from the date of the examination. Valid physicals are necessary for participation in all athletic activities including summer events and camps. Families can choose the medical facilities they wish to have the physical performed.  Please pick up your packet from the high school office or print off from the "Athletics" tab on the school website as they are the complete physical packet. Forms that are picked up at a doctor’s office or are printed off the OHSAA website are not complete for the school’s purposes as they don’t have the paperwork from the school.  Turning in an incomplete packet will result in delayed participation.  Go Bulldogs!
