STATUS 

• 50% of the families in Q1 2023 of New Families received a home visit.  The Family Connects program has had a successful quarter with positive feedback from residents who have received the program. The team is addressing challenges with data collection and workflow and working on raising awareness about the program with partners and community members. There is a possibility of an increase in eligible birth population as information is collected from vital statistics. The team is grateful for the skilled staff providing high-quality visits in the home and excited to have this new program in the community.
