Google Forms

Have you ever used Google Forms with your students? Parents? Colleagues? Boy are you in for a treat! Use this webpage to:

            • Learn to easily collect information from students, parents, and colleagues.
            • Gain ideas on ways to use Google Forms in your classroom.
            • Learn to create a Google Form.

Login to Google

Teacher:

Username: DISD Email

Password: DISD Password (password will automatically change as you change it throughout the year)

Student:

Username: xxfirst.mi.last@dvilleschools.org

Password: DISD Password (password will automatically change as you change it throughout the year)


xx's = last 2 digits of student's graduation year.

mi = middle initial

Example: 17John.R.Smith@dvilleschools.org

From Google Drive





  1. Create a new from by selecting NEW.
  2. Then select More.
  3. Google Forms.

Create a new from by selecting the BLANK template or choose one of the premade templates shown.

Google Quiz


Google Forms now grades quizzes for you! This feature only expresses how many answers were correct out of the total possible points.

All you have to do is...

  • Create a new form.
  • In settings, check Make this a quiz.
  • Add questions.
  • Create an Answer Key.
  • Try it out!

Step-by-step instructions on creating a quiz.

  1. Select Settings (the Gear icon).
  2. On the General tab select options needed for your quiz. If you select "Collect email addresses" and "Response receipts" it allows the students to get the answers they submitted.
  3. On the Presentation tab select options for your quiz.
  4. On the Quizzes tab select, Make this a quiz.
  5. Select quiz options and what the respondent can see.

If you choose to Release Grade (in the Settings/Quizzes tab) immediately after submission, it gives students an option to view their score at the end.

If you leave all of the items under Respondent can see (in the Settings/Quizzes tab) then students will see something similar to the image above. Thus, allowing opportunities for correct answers sent to others students that haven't taken the exam.

Create an Answer Key





  1. While editing question, select Answer Key at the bottom of the question.
  2. Select the correct answer.
  3. Assign points to the question.
  4. Select, EDIT QUESTION when finished.

View Your Score

After entering all the questions and answers, it is now time to generate the spreadsheet where all of the submissions will be captured.


Creating the Spreadsheet

1. At the top of the quiz, you should see a Questions tab and a Responses tab. Select the Responses tab.

2. Select the green spreadsheet icon. (Shown in picture below on the left.)

3. Create a new spreadsheet, should be selected. Click on CREATE. (Shown in picture below on the right.)

This will create a spreadsheet in the same folder as the form.

After students have finished their quiz, you can view their scores by opening the spreadsheet. This can be found in the same folder where the form is located.

Receive email notifications for new responses




Select the 3 dots to launch the menu to select the option to receive email notifications for new responses.
