SOCIAL WELFARE OFFICER II
Assist the Field Office in complying with the trust, directives an plans prescribed by the FO by responding to queries, conduct monitoring, provide technical assistance and guidance to LGUs on SFP operations concerns ensuring timely implementation;
Prepare technical reports based on monitoring and technical assistance provided to LGUs reference on the provision services to beneficiaries;
Review, consolidate and monitor physical accomplishments of the assigned province in coordination with the LGU focal person and FOs;
Submit monthly/ quarterly consolidated accomplishment report of assigned area which includes monitoring and evaluation of SFP reports submitted by LGUs with analysis and recommendation;
Prepare, consolidate and submit documentation reports of LGUs which includes documentation of good practices, augmentation support of LGUs to SFP funds and other significant accomplishments;
Coordinate with the LGUs for the timely submission of SFP reports in relation to funds and other significant accomplishments;
Attends to meetings, consultation and coordination with partner stakeholders and concerned agencies; and,
Performs other project related tasks that may be assigned by the Regional Focal Person.
Quarterly/ semestral. Annual SFP statistical and narrative report;
Consolidation of the Monthly Physical/ Financial Accomplishment of provinces;
Case Management Report on target severely undernourished beneficiaries;
Reports on the conducted monitoring, evaluation and provision of technical assistance to partner LGUs and other stakeholders with analysis and recommendation;
Documentation Reports of LGUs which includes documentation of good practices augmentation support of LGUs to SFP funds and other significant accomplishments

Comprehensive Resume or Hard copy of the Personal Data Sheet (CS Form No. 212 with an attached job description for each position held in previous work);
Transcript of Records and Diploma (Certified photocopies);
Certificates of Trainings / Seminars attended and Employment;
Certificate/s of Board Rating/ Professional License/ CS Eligibility, if applicable only
Certification from school for Masteral units earned or completed, if any, and
Individual Performance Contract Rating for the last semester of employment for applicants from any government agencies; or Performance Evaluation signed by the supervisor or the HR Head for applicants from any private company/ institution; or GMC for newly graduates.
