Education : Bachelor’s Degree relevant to the job
Training : Four (4) hours of relevant training
Experience : One (1) year of relevant experience
JOB FUNCTIONS
Prepares plan of activities and budgetary requirements at the municipal level.
Plans and organizes meetings, assemblies and other activities and ensure that supplies, materials, venues, facilities are available and personnel and required attendees (like Beneficiaries, partner representatives from LGU, DepEd, DOH and other resource persons) are present
Conducts and/or facilitates meetings and assemblies to ensure that roles, tasks, expectations are clear and leaders and focal person are identified.
Organizes and mobilizes the stakeholder partners (health center physicians, teacher/principal, LGU Link) in accordance with program requirements.
Records the proceedings of meetings and assemblies, documenting issues, concerns and gaps identified resolved/unresolved and corresponding recommendations and train and assign other record and document as well.
Carries out clerical and administrative work in the Municipal Assembly Committee/Local Assembly Committee.
Records and updates municipal caseloads as follows:
Total registration and enrollment
Number of Household with cashcards
Number of Households on otc
Number of Households on gc
Updated list of schools – day care, elementary, highschool
Updated list of health centers
Number of Parent Leaders
Number of Household provided with other support programs and services.
Prepared monthly Accomplishment Report and all other reports
Leads the LGU Links/Municipal Social worker and SWA in the distribution to, and retrieval of SSA forms from, health and education counterparts.
Leads the LGU Links/Municipal Social Worker and SWA in the distribution to health and education counterparts and retrieval of CVS and Update forms AND SSA Forms
Attends to complaints and identifies potential/real grievances, investigates and facilitates resolution of grievances that were referred to grievance focals
Prepares and coordinates logistics and personnel requirements for the pay-out and ensures that proper procedures are observed; records payment turn-outs, and prepares corresponding reports and recommendations
Conducts and/or facilitates the conduct of family development sessions(FDS) and evaluates the same to develop appropriate responses
Logs issues and concerns and identifies those requiring case management and endorses these to SWA, MSW and SWO3.
Checks and records attendance of grantees, investigates absentees and reports/endorses to SWA/MSW/Cluster Link for appropriate action
Conducts roll-out training and capability building of parent leaders and ensures all topics are discussed. Clarifies/resolves and records issues and concerns
Checks and records the attendance of parent leaders and updates their profiles and trainings attended.
Attends municipal/cluster/provincial meetings and evaluation workshops (like mid-assessment, PREW) and training and development activities
Prepares Performance Matrix for SWA
