College Admission

Download and accomplish College Admission Application Form. Make sure all applicable entries are complete and the accomplished form is duly signed. Write N/A if a field is not applicable.

Scan the accomplished application form using flatbed scanner or document scanning mobile application (e.g. ClearScan, Adobe Scan). Save it in PDF format and assign a filename using the format <Lastname><Firstname>_NewStudent.pdf (e.g. DelaCruzJuan_NewStudent.pdf).

The following documents must be submitted together with the duly accomplished College Admission Application Form

• Scanned copy (in PDF format) of Form 138 (Report Card) with at least 2nd grading period. Use filename format <Lastname><Firstname>_Form138.pdf, e.g. DelaCruzJuan_Form138.pdf.

• Softcopy of 2x2 photo with nametag and in white background in JPG file format. Use filename format <Lastname><Firstname>_2x2.jpg, e.g. DelaCruzJuan_2x2.jpg.

Attach all the above-mentioned requirements and send/email to DMMMSU-OUS Admission Office using the following details:

• Softcopy of 2x2 photo with nametag and in white background in JPG file format. Use filename format <Lastname><Firstname>_2x2.jpg, e.g. DelaCruzJuan_2x2.jpg.
