By filling out the Pre-Registration form, you hereby agree to the terms and conditions as defined below.

• I understand that this application does not mean I am officially enrolled and further agree to the conditions for admission.

• I declare that I am providing true and complete details about my personal information and required data.

• I agree to submit hardcopy of my Honorable Dismissal and other admission requirements to DMC Registrar's Office.

• I will wait for a confirmation call from the registrar's office to proceed next step of my enrollment.

• I am aware that my ₱ 1,000.00 Payment for Upon Registration is NON-REFUNDABLE.  My ₱ 1,000.00 will be deducted to my tuition fee once officially enrolled. (Click here to see payment details)
