Newly admitted regular student
Filipino citizen and resident of the Philippines
W/ Highest Honors and w/ High Honors of the graduating batch with a minimum of 50 total number of graduating students and more.
W/ General Weighted Average (GWA) of 95% above
Belongs to the top twenty (20) examinees of the DLSMHSI Entrance Scholarship Qualifying Exam (for incoming Senior High, BS Nursing, and BS Medical Laboratory Science)/top ten (10) (for other undergraduate programs offered).

Scholarship Application Form for Entrance Scholarship-UG. - for incoming Undergraduate
Upload and fill out the following requirements on the assigned Google Drive (Please rename the files to be uploaded as (Last Name, First Name, Middle Name & Temporary/Student ID number) & in pdf format).
Requirements:
Completed and signed Entrance Scholarship Recommendation Form by the School Principal / Director certifying status as W/ Highest Honors and w/ High Honors of the graduating batch with 50 total of graduating students and more.
High School Report Card with General Weighted Average (GWA) / General Point Average (GPA).

Entrance Scholarships are automatic but the applicants are required to file a Scholarship Application Form and submit its requirements on or before the application period.
Entrance Scholarship applicants may submit their application pending the other requirements. However, applicants can only avail of your tuition fee discount after the applicant completes the required documents. The deadline for pending requirements is on or before July 25, 2025.
