Epic Opt out letter for Parents in Spanish

Logging into and creating accounts for students requires sharing private and personal student information. District 65 requires a data sharing agreement with a company that needs this type of information. We do not have a data sharing agreement with Epic (getepic.com). However the district realizes that this tool is in wide use and supports units of study. Therefore we ask that you use an "OPT OUT" letter for this resource, should parents decide their child needs to opt out of using this tool. You can find a letter for EPIC! opt our here.

Please do not think of this or communicate this as an option for other tools that we do not have a data sharing agreement with. Teachers do not have permission to copy the wording and use for other systems. This not not the best way to get permission for tools but we are willing to do this for EPIC! due to the conditions stated above.

Teachers and/or librarians (work together by school ) to send to the families who are being asked to use the tool. The Google form will be managed at the district level. A translation of the opt out letter is being worked on now and we'll share as soon as it becomes available.
